NOTICE OF RETIREMENT Dr Kem ALGU

It is with great sadness that I wish to advise you and your family of my retirement on 30 November 2021. It has been my absolute pleasure and privilege to have known each one of you and your families. Thank you for trusting me to take care of your health needs over the past twenty-one years. I will continue to be available to care for you until the date of my retirement.

FINDING A NEW FAMILY PHYSICIAN:
I am pleased to advise you that we are fortunate to have doctors new to our practice who are accepting limited number of patients. It will be advisable that you call our office at 780-400-3322 to schedule an introductory visit. Please note that many family physicians are not comfortable treating patients with non-cancer chronic pain on opioids (including codeine and tramadol) or those patients on benzodiazepines/zopiclone/eszopiclone/zolpidem. Please see below for additional information that may help you to find a new family physician.

Medical practices frequently change as does the list of clinics accepting new patients. You are encouraged to reach out to the following resources to find up to date information on family physicians accepting new patients, or for support with finding a new family physician:

ACCESSING YOUR MEDICAL INFORMATION:
Your medical records will remain in the clinic unless your written authorization is received to transfer them to another physician outside our clinic, as is required by the CPSA. Your new family physician may want a copy of your medical records, particularly if you had seen other specialists or have a complex medical history. Your new family physician may fax the authorization to 780-449-6993. If you remain within our group of clinics, then no request is necessary.
You may also access some of your health records at myhealth.alberta.ca/myhealthrecords
My Health Records is an Alberta Health online tool that you can use to view your immunizations, medications, results of common lab tests and diagnostics. Your physician can access these records and more on NETCARE.

You may also access some of your medical records on our patient portal. If you have not signed up to our portal, please discuss with our reception staff the next time you are in the clinic. Access to our portal is free but requires an email address. The portal allows you to have limited communication with your physician at our clinic, gives you access to some medical consultation reports, and allows you to make online appointments.

In closing, I want to thank each one of you for allowing me and our staff to be part of your health care team. It has been my privilege and pleasure to provide medical care for you and your family. It has been an immense honour to get to know you, and you have made my medical career rich and rewarding. I will look back on this time with much warmth and many fond memories thanks to each of you.
